By completing this service order form and signing the credit card authorization, I authorize Family Care Options, Inc. to charge the credit card provided for all services rendered. I understand that a 4% service fee applies to telephone and online transactions.
This Stroller Rental Agreement ("Agreement") is entered into between [Family Care Option] ("Company") and the undersigned renter ("Renter"). 1. Assumption of Risk and Release of Liability The Renter acknowledges and agrees that the Company is not liable for any injuries, accidents, or damages sustained by any person arising from the use of the stroller. The Renter assumes all risks associated with use of the stroller.
2. Responsibility for Damage or Loss The Renter is responsible for returning the stroller in the same condition it was received, normal wear and tear excepted. - If the stroller is returned damaged, lost, or stolen, the Renter will be charged the full replacement cost of the stroller. - Charges will be processed to the credit card on file.
3. Rental Period The stroller must be returned by the agreed-upon date and time. Late returns may be subject to additional rental fees.
4. Governing Law This Agreement shall be governed by the laws of the state in which the rental occurs.
RATES: Based per day, per stroller.

Luxury Single Stroller: $69.00
Double Stroller: $69.00
Luxury Double Stroller: $79.00
Jogging Stroller: $69.00
Luxury Jogging Stroller: $79.00
Umbrella Stroller: $49.00
Pet Stroller: $59.00
Luxury Pet Stroller: $69.00
Delivery Charge: $45.00 Round trip personal door-to-door delivery
Cancellation Policy:
Cancellations received less than 48 hours before the scheduled start time, the client will be responsible and billed for the entire schedule of services confirmed. All hourly rates and transportation fees will be charged to the credit card provided.
Cancellations received more than 48 hours in advance will receive a refund.
Payment & Confirmation: Upon submission, services will be confirmed, and the credit card provided will be charged. All major credit cards are accepted.
