I understand and agree by signing below that when I hire a permanent caregiver referred to me by Family Care Options, I will be responsible to pay a permanent placement fee of 15% to Family Care Options and the fee is based on the caregivers yearly net salary. My intentions are to hire a permanent caregiver from Family Care Options and once the placement fee is paid to Family Care Options, I will become the caregivers direct employer responsible for any and all tax due to the government as the employer. I also understand and agree that the information I provided above is true information and my intention is to provide a safe workplace for the caregiver.
Family Bliss Bundle: $249.00 fee for Nanny & Stroller Rental
Surchage Policy: A $50.00 surcharge fee applies before 8:00 AM and/or after 9:00PM.
Cancellation Policy:
Cancellations received less than 48 hours before the scheduled start time, the client will be responsible and billed for the entire schedule of services confirmed. All hourly rates and transportation fees will be charged to the credit card provided.
Cancellations received more than 48 hours in advance will receive a refund.
Payment & Confirmation: Upon submission, services will be confirmed, and the credit card provided will be charged. All major credit cards are accepted.
